About the role

Key responsibilities & impact
  • Assess and evaluate members' behavioral health needs
  • Guide members and families toward appropriate care and wellbeing resources
  • Facilitate interaction between members and support resources
  • Employ strategies and techniques to manage physical, environmental, and psychosocial health issues
  • Resolve barriers that hinder care
  • Manage patient care through assessments and evaluations
  • Ensure patients progress toward desired outcomes
  • Create member care plans as needed
  • Interpret and independently determine appropriate courses of action
  • Report to the Manager, Care Management Behavioral Health
  • Perform mandatory field work, including onsite/home member visits, traveling up to 50% of the time
  • Work a combination of remote work at home and onsite/home member visits
  • Understand department, segment, and organizational strategy and operating goals

Requirements

What you’ll need
  • Must have an active, unrestricted SC Licensed Masters Social Worker (LMSW), Licensed Professional Counselor (LPC) OR an active, unrestricted compact Registered Nurse (RN) license
  • 2 or more years of experience working as a behavioral health professional
  • Reside in the state of South Carolina and willing to travel
  • Have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
  • Experience with behavioral change, health promotion, coaching and wellness
  • Proficient in electronic information systems and Microsoft Office tools (Word, Outlook, Excel)
  • Experience supporting patients telephonically preferred
  • Experience working in Community Mental Health, crisis response, Assertive Community Treatment (ACT), or substance use disorder treatment preferred
  • Experience working with both children and adults preferred
  • Must be able to work a 40-hour work week, Monday through Friday, 8:00 AM to 5:00 PM
  • Must meet home internet requirements of at least 25 Mbps download and 10 Mbps upload
  • Must work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information
