Key responsibilities & impact

Assess and evaluate members' needs and requirements to achieve or maintain optimal wellness
Guide members and families toward appropriate care and wellbeing resources
Facilitate interaction with care and community resources
Manage members' physical, environmental, and psychosocial health issues using varied strategies and techniques
Identify and resolve barriers to effective care
Monitor patient care through assessments and evaluations
Ensure patients progress toward desired outcomes
Create member care plans as needed
Apply department, segment, and organizational strategy and operating objectives
Travel up to 50% to support collaboration, conduct face-to-face meetings, and engage with staff, providers, members, and families
Report to the Manager, Care Management of Behavioral Health

What you’ll need

Bachelor's in nursing (BSN)
Active license in the state of South Carolina without disciplinary action
Must reside in the State of South Carolina
2 or more years of experience in case/care management
2 or more years working with the behavioral health population
Knowledge of community health and social service agencies and additional community resources
Ability to use electronic information applications/software programs, including electronic medical records
Reside close to the Midlands OR Upstate area where your region will be
Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speed
Preferred: 3 or more years of in-home assessment and care coordination experience
Preferred: Experience with health promotion, coaching and wellness
Preferred: Previous managed care experience
Preferred: Bilingual — English, Spanish
Preferred: Certification in Case Management
Preferred: Motivational Interviewing Certification or knowledge
