About the role
Key responsibilities & impact- Identify and resolve barriers that hinder effective care
- Continuously monitor patient care through assessments and evaluations
- Ensure members progress toward desired outcomes
- Create plans of care
- Communicate with internal and external stakeholders
- Apply department, segment, and organizational strategy and operating objectives
- Follow established guidelines and procedures
- Visit Medicaid members in homes, Supportive Living Facilities, Long-Term Care Facilities, and other care settings
- Travel 75–90% locally in the field
- Attend occasional onsite work at the market office in Schaumburg, Illinois
- Travel occasionally to Humana offices for training or meetings
Requirements
What you’ll need- Must reside in Franklin, Union, Jackson, Johnson, Massac, Hamilton, White, Alexander, Williamson, Saline, Hardin, Pope, Gallatin, or Pulaski County, Illinois, within the listed ZIP codes or within a 10-mile radius
- Registered Nurse (RN) in the state of Illinois without disciplinary action
- Clinical nursing experience required, including hospital, acute care, or home health experience
- Ability to travel 75–90% within Illinois
- Knowledge of community health and social service agencies and additional community resources
- Ability to use electronic information applications/software, including electronic medical records
- Valid state driver's license
- Maintain personal vehicle liability insurance meeting the stated minimum limits or residing-state requirements
- Ability to work Monday–Friday, 8am–5pm CST
- Must complete tuberculosis (TB) screening if selected
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload for home or hybrid employees
- Work from a dedicated space without ongoing interruptions to protect PHI/HIPAA information
- Preferred: Bachelor of Science in Nursing (BSN)
- Preferred: 2+ years of in-home case/care management experience
- Preferred: Experience with Medicare/Medicaid members
- Preferred: Experience with health promotion, coaching, and wellness
- Preferred: Previous managed care experience
- Preferred: Bilingual English and Spanish
- Preferred: Certification in Case Management
- Preferred: Motivational Interviewing Certification and/or knowledge in the area
