About the role

Key responsibilities & impact
  • Assess and evaluate members’ needs and requirements to support optimal health, safety, independence, and quality of life
  • Guide members and families in accessing appropriate care, resources, and services
  • Apply person-centered, trauma-informed, and culturally responsive strategies to functional, behavioral, environmental, and psychosocial needs
  • Manage members’ physical, environmental, and psychosocial health issues
  • Identify and resolve barriers that hinder effective care
  • Monitor member care through assessments and evaluations to ensure progress toward desired outcomes
  • Create member care plans as needed
  • Support eligibility determinations, service planning, risk identification, and care coordination
  • Collaborate with interdisciplinary teams, health care providers, members, caregivers, and community-based organizations
  • Maintain compliant documentation
  • Travel 75%–90% within Wayne and/or Macomb Counties, Michigan
  • Work Monday through Friday, 8:30 AM–5:00 PM Eastern Time

Requirements

What you’ll need
  • Must reside in Michigan and be able to travel throughout the assigned service area, including Wayne and/or Macomb Counties
  • Active Michigan Social Work license in good standing required (LBSW, LLMSW, or LMSW); candidates with a limited license must demonstrate active progress toward full licensure
  • 2+ years of post-degree experience in social work, behavioral health, health care, care management, or community-based services
  • 1+ year of experience supporting Home and Community-Based Services (HCBS) and/or waiver programs
  • Experience conducting Long-Term Services and Supports (LTSS) assessments
  • Ability to assess functional, behavioral, environmental, and psychosocial needs for eligibility determinations, service planning, risk identification, and care coordination
  • Knowledge of LTSS, HCBS, Medicaid requirements, person-centered planning principles, member rights, and least restrictive settings
  • Experience collaborating with interdisciplinary teams, health care providers, members, caregivers, and community-based organizations
  • Knowledge of community health, behavioral health, social service agencies, and local resources
  • Ability to maintain accurate, timely, and compliant documentation according to organizational, state, and federal requirements
  • Strong verbal and written communication skills
  • Proficiency with electronic medical records, care management platforms, and health care technology applications
  • Strong computer skills, including Microsoft Word, Outlook, Excel, and web-based applications
  • Fluent in English and Arabic or Chaldean, with the ability to speak, read, and write each language independently and without assistance (preferred)
  • Must complete TB screening
  • Valid state driver’s license and personal vehicle liability insurance required
