About the role

Key responsibilities & impact
  • Assess and evaluate members' needs and requirements to support optimal wellness
  • Meet members in their locations and assess needs, barriers, goals and priorities
  • Conduct initial and ongoing long-term services and support (LTSS) assessments using person-centered thinking approaches
  • Contact members telephonically and/or in person
  • Develop and continuously modify service plans
  • Involve care-team members in care planning
  • Identify LTSS providers and community partnerships
  • Support members with LTSS, environmental, social, housing, educational and other services
  • Assist members in maintaining Medicaid eligibility
  • Prevent duplicative assessments using available member information
  • Collaborate with Medical Directors, geriatricians and care coordinators to ensure cohesive service delivery and positive outcomes

Requirements

What you’ll need
  • Bachelor's degree in health and human services field
  • 2 or more years of related experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Must reside in Cass, Fulton, Howard, Miami, Tipton, or Wabash County, Indiana
  • Experience with electronic case notes documentation and multiple computer applications/systems preferred
  • Experience with health promotion, coaching and wellness preferred
  • Knowledge of community health and social service agencies and community resources preferred
  • Home internet with at least 25 Mbps download and 10 Mbps upload
  • Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
