About the role
Key responsibilities & impact- Provide case management and support to adults in the community
- Assure quality, cost-effective services while monitoring members' health and safety
- Work as part of an Interdisciplinary Team (IDT) for Family Care Partnership members
- Conduct home and community visits
- Complete nursing assessments and provide input to members' care
- Monitor wellness and health promotion activities
- Re-evaluate members' health and wellness and responses to interventions
- Perform routine health monitoring activities
- Build working relationships with members, families, and outside agency staff
- Assess psychiatric, behavioral, or medical problems during crises and implement interventions
- Evaluate members' ability to manage medications, medical interventions, equipment, and healthcare access
- Provide or arrange services, supports, or medical care and monitor effectiveness
- Assist with program, policy, and procedure development
- Provide input for the QI/QA program
Requirements
What you’ll need- Must be a Registered Nurse, licensed in the State of Wisconsin
- Two (2) years of related health care experience
- Must be able to travel to any location within Racine, Kenosha, Waukesha, Washington, Ozaukee, Walworth, and Sheboygan counties
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Must complete TB screening if selected
- Dedicated work space without ongoing interruptions to protect member PHI/HIPAA information
- Preferred: Bachelor of Science degree in Nursing (BSN)
- Preferred: Previous home health, managed care, or case management experience
- Preferred: Previous experience working with persons with physical, intellectual/developmental disabilities, or older adults in a health care setting
- Preferred: Case Manager Certification
- Preferred: General knowledge of Medicaid and Medicare benefits
- Home internet with at least 25 Mbps download and 10 Mbps upload speeds
