About the role
Key responsibilities & impact- Complete the nursing portion of comprehensive member assessments and provide care recommendations
- Monitor wellness and health promotion activities based on member assessments and identified outcomes
- Re-evaluate members' health and wellness to manage ongoing issues and assess responses to interventions
- Perform routine health monitoring activities based on member needs
- Build working relationships with members, families, and outside agency staff
- Assess psychiatric, behavioral, or medical problems during crises and implement interventions
- Evaluate members' ability to manage medication, medical care, treatments, equipment, and healthcare access safely
- Provide or arrange services, supports, or medical care that meet identified needs
- Monitor the effectiveness of care interventions
- Assist with program and procedure development and QI/QA program input
- Conduct home and community visits
- Travel within Milwaukee and surrounding counties
Requirements
What you’ll need- Must be a Registered Nurse, licensed in the State of Wisconsin in good standing
- Two (2) years of related health care experience
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Must complete TB screening if selected
- Self-provided internet service with at least 25 Mbps download and 10 Mbps upload speeds
- Dedicated workspace without ongoing interruptions to protect member PHI / HIPAA information
- Preferred: Bachelor of Science degree in Nursing (BSN)
- Preferred: Previous home health, managed care or case management experience
- Preferred: General knowledge of Medicaid and Medicare benefits
