About the role
Key responsibilities & impact- Assess members’ health and safety needs to develop comprehensive Member Care Plans
- Conduct regular health assessments
- Coordinate with acute and primary care providers
- Educate members on disease processes, risks, nutrition, exercise, and lifestyle modifications
- Document in member files within required DHS contract timeframes
- Participate in monthly member phone contacts and travel to in-person visits
- Complete record reviews for reassigned members and identify remediation needs
- Support record review and remediation activities
- Complete remediation documentation
- Inform newly assigned RNs and Care Coach partners of missing or needed follow-up
- Assign tasks in the D365 system
- Accept additional complex member assignments
- Support new member assignments
- Travel approximately 40% of the time to visit members
- Provide care coordination, support, and education to Wisconsin Family Care Program members
Requirements
What you’ll need- Associate degree in nursing
- Licensed Registered Nurse in the state of Wisconsin (in good standing)
- Valid state driver's license
- Proof of personal vehicle liability insurance with at least 100/300/100 limits
- Ability to work Monday through Friday, 8:00 am–4:30 pm CST
- Ability to work remotely from a dedicated space without ongoing interruptions
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload
- Patient-facing role requiring TB screening if selected
- Reside within 45 minutes or less of the assigned coverage area (preferred)
- 3 years Care Management experience (preferred)
