About the role

Key responsibilities & impact
  • Assess and evaluate members’ needs and requirements to support optimal wellness
  • Engage and empower members and families using holistic strategies for recovery, health maintenance, and wellness participation
  • Conduct telephonic and in-person assessments
  • Guide members and families toward appropriate care and community resources
  • Monitor member progress through regular assessments and evaluations
  • Develop and implement personalized care plans and clinical interventions
  • Collaborate with healthcare providers, community organizations, and service providers
  • Coordinate essential services addressing physical health needs and social determinants of health
  • Facilitate communication across interdisciplinary care teams and primary care providers
  • Document and submit incident reports in accordance with organizational policies and regulatory requirements
  • Report to the Manager of Care Management within the Louisiana Medicaid Team

Requirements

What you’ll need
  • Must reside within Louisiana Regions 1, 2, 3, 4, or 8
  • Active, unrestricted Registered Nurse (RN) license in Louisiana
  • Two (2) or more years of clinical experience working with diverse populations, including youth, adults, and geriatric populations
  • Knowledge of community health and social service agencies and community resources
  • Experience with electronic medical records and other electronic information applications
  • Excellent keyboarding and web navigation skills
  • Ability to provide narrative documentation
  • Intermediate to advanced computer skills
  • Experience with Microsoft Word, Outlook, and Excel
  • Ability to work independently with minimal supervision
  • Ability to exercise sound judgment within the scope of the role
  • Ability to proactively adapt care plans to changing member needs and circumstances
  • Monday through Friday availability, 8:00 AM to 5:00 PM CT
  • Ability to travel up to 50% within the assigned Louisiana region
  • Ability to work from a dedicated space without ongoing interruptions and maintain required internet speeds (25 Mbps download, 10 Mbps upload)
