About the role

Key responsibilities & impact
  • Perform telephonic and face-to-face assessments and evaluations for long-term services and support (LTSS) and aged, blind and disabled (ABD) members
  • Meet members in the field at their homes or locations of their choice
  • Assess members’ goals, needs and barriers
  • Connect members and families with appropriate quality services and community resources
  • Review, analyze, assess and attest for medical complexity
  • Develop individualized member care plans
  • Apply evidence-based strategies to manage members’ physical, environmental and psychosocial health
  • Identify and resolve barriers to effective care
  • Continuously monitor member care and progress toward desired outcomes through comprehensive assessments and evaluations
  • Make autonomous decisions in varied and occasionally ambiguous situations
  • Follow established guidelines and procedures
  • Travel 50–75% for field interactions with members, families and providers
  • Occasionally attend onsite meetings at the Humana Healthy Horizons office in Glen Allen, Virginia
  • Support members with person-centered health outcomes

Requirements

What you’ll need
  • Must reside in the Central Region of the Commonwealth of Virginia in the counties/independent cities of Richmond, Henrico or Midlothian areas
  • Active Registered Nurse (RN) license in the Commonwealth of Virginia without disciplinary action
  • 2 years of prior experience in health care and/or case management
  • 1 year of experience working with individuals at risk for chronic medical conditions and high social needs, including adult and pediatric populations
  • Knowledge of community health and social service agencies and additional community resources
  • Intermediate to advanced computer skills
  • Experience with Microsoft Word, Outlook, Excel, electronic information applications/software programs, and electronic medical records
  • Excellent keyboard and web navigation skills
  • Exceptional communication and interpersonal skills with the ability to quickly build rapport
  • Ability to travel to region-based facilities and homes for face-to-face assessments and interactions with members and/or families
  • Ability to work Monday–Friday, 8:00am–5:00pm Eastern Standard Time
  • Preferred: Bachelor of Science in Nursing (BSN)
  • Preferred: Certification in Case Management (CCM)
  • Preferred: Previous managed care experience
  • Preferred: Experience with in-home assessment and care coordination
  • Preferred: Experience with disease management, health promotion, coaching and wellness
  • Preferred: Bilingual or multilingual ability in English/Spanish, Arabic, Vietnamese, Amharic, Urdu or another language; must speak, read and write both languages without limitations and assistance
  • Must complete the Interagency Language Rating (ILR) test if speaking with members in a language other than English
