About the role
Key responsibilities & impact- Manage members' health issues using telephonic strategies, approaches and techniques
- Identify and resolve barriers to effective care
- Conduct assessments and monitor patient care and progress toward desired outcomes
- Use data, conversations with members, and active care planning to support care
- Guide members and their families to essential health and community resources
- Apply professional concepts, standards, regulations, strategies and operating standards
- Make decisions regarding work approach and priorities while following direction and established procedures
- Report to the Manager, Care Management
- Join a dedicated team supporting compassionate, patient-centered service
Requirements
What you’ll need- Bachelor's degree in a health-related field, active Illinois LPN licensure, or the ability to obtain an IL LPN license
- 2 or more years of case management, medical case management, care coordination, or healthcare support experience
- Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
- Previous experience with electronic case note documentation
- Experience documenting in multiple computer applications/systems
- Experience with health promotion, coaching and wellness
- Knowledge of community health and social service agencies and additional community resources
- Ability to work Monday–Friday within operating hours of 8:00 AM–7:00 PM CST, with core hours primarily 8:00 AM–5:00 PM CST
- Minimum home internet speed of 25 Mbps download and 10 Mbps upload
- Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
