About the role

Key responsibilities & impact
  • Supervise, coordinate, and support implementation of Social and Community Services Programs
  • Build and manage a high-performing team
  • Plan, assign, schedule, and review team work while adhering to quality standards
  • Hire, train, develop, conduct performance reviews, and administer corrective action when applicable
  • Set professional goals and address barriers to members’ care
  • Develop processes and workflows that improve member and team-member experience
  • Support smooth transitions of care and member wellbeing
  • Educate, coach, and support the SCS team in partnership with hospitals and providers
  • Discuss complex cases, assist with escalated member needs, and recommend further action
  • Develop agendas and lead monthly team meetings
  • Track strategic goals, outcomes, KPI data, productivity, and HEDIS compliance contributions
  • Communicate field intelligence to the SCS Manager
  • Resolve service issues and represent Community Health and IEHP leadership
  • Liaise with SCS teams, provider offices, and community-based organizations
  • Nurture relationships with healthcare providers, community organizations, and stakeholder partners
  • Work with providers, community organizations, and leaders to improve quality metrics and PHM Bold Goals 50x2025
  • Attend and represent the SCS department at IEHP meetings and external events
  • Coordinate special projects with the SCS Manager and community partners
  • Share best practices, strengthen education and outreach, and promote care connections and disease prevention
  • Conduct chart audits, quality assurance checks, coaching, clinical support, and shadowing
  • Assist with closing complex and escalated member cases
  • Facilitate regular case conferences
  • Identify performance issues, care gaps, and operational efficiency opportunities
  • Train team members on IEHP policies, regulations, procedures, workflows, and electronic medical management systems such as MedHok
  • Monitor continuous quality improvement involving healthcare access, primary care, prevention, social determinants of health, and HEDIS gaps
  • Implement the LEAN model for program standards, processes, and training
  • Act on behalf of the Manager when needed
  • Attend meetings, provide training, and perform next-level representation as required

Requirements

What you’ll need
  • At least three (3) years of experience in supervision/management, managed care/IPA/clinic setting, or delivering presentations/trainings to varied audiences
  • Experience with State Health Programs (Medi-Cal/Medicare), case coordination, community health work/Promotora programs, community outreach, behavioral health/substance use disorder programs, case management, or acute care
  • Experience leading a team and operations with accountability for outcomes
  • Master’s degree in Social Work from an accredited institution required
  • Active, unrestricted, and unencumbered LCSW license issued by the California Board of Behavioral Sciences required
  • Knowledge of Medicare, Medi-Cal, HEDIS, MCAS, DHCS, and NCQA required
  • Knowledge of social determinants of health, community resources in San Bernardino/Riverside counties, healthcare services, CHW roles, and multicultural communities
  • Bilingual in one of IEHP threshold languages is highly desirable: English, Spanish, Chinese, or Vietnamese
  • Excellent verbal, communication, and interpersonal skills
  • Motivational interviewing or similar empathy-based communication skills
  • Computer skills including typing, data entry, email, Word, Excel, Outlook, and PowerPoint
  • Planning, organization, time management, critical thinking, analytical, problem-solving, and collaboration skills
  • Ability to organize and prioritize workload, work under pressure, work independently, and maintain attention to detail
  • Ability to engage team members by phone, in members’ homes, in the community, and other settings
  • Ability to develop relationships with community-based organizations and leaders
  • Must be able to lift 25 lbs.
  • 75% driving travel required in assigned Riverside and San Bernardino County territories
  • Must be willing and able to travel to members’ homes
  • Must be willing to work flexible schedules including evenings and weekends when needed
