About the role
Key responsibilities & impact- Organize and collect information from patients during follow-up telephone contacts
- Track and report individual and aggregate clinical information
- Apply positive communication skills with internal and external customers
- Deliver cost-effective, high-quality care
- Facilitate a systematic approach to surveillance and educational outreach
- Assemble and review clinical records from physician offices, patients, and internal data systems
- Forward patients outside the defined acceptable range to a Pharmacist for case review
- Use pharmacist-defined assessments to screen for potential issues and report issues to a pharmacist
- Contact patients to obtain accurate and timely information, facilitate positive outcomes, and determine therapy tolerance
- Document and report adverse events, adverse effects, and lack of tolerability to physicians, pharmacy, and management
- Use CPR+, MS Office, Word, Excel, and PowerPoint to record data and create reports
- Assist with maintenance of Prior Authorization forms
- Maintain positive communication and resolve difficult situations
- Follow applicable pharmacy and nursing laws and established procedures
Requirements
What you’ll need- High School diploma or GED required
- Certified Pharmacy Technician or NYS licensed LPN preferred
- Three (3) years of pharmacy experience required
- Demonstrated ability to identify barriers to a successful care management path
- Demonstrated ability to interact effectively with physicians and other members of the health care system
- Proficient PC and Windows skills, including MS Office
- Excellent problem-solving abilities and time management skills
- Excellent written, verbal and interpersonal communication skills
- Demonstrated transferable knowledge, skill and ability to complete job duties independently and proficiently
- Flexibility in work schedules and assignments
- Proven examples of displaying the Reliance values: Collaborative, Accessible, Results-Oriented, Empowering, and Supportive
- Must comply with applicable Federal, NYS, and other state pharmacy law, and nursing law if applicable
- Immigration or work visa sponsorship will not be provided
