About the role

Key responsibilities & impact
  • Collaborate with patients by assessing, facilitating, planning, and advocating for individual health needs
  • Track referrals and maintain appropriate documentation to promote team awareness and patient safety
  • Complete accurate patient registration, including demographic and current insurance information
  • Assemble clinical background and referral-needs information and provide appropriate clinical information to specialists
  • Contact review organizations and insurance companies to meet prior-approval requirements
  • Present medical information such as history, diagnosis, and prognosis
  • Provide medical information to financial services to maximize hospital and physician reimbursement
  • Review referral details and expectations with patients
  • Help patients address healthcare-system, financial, and social barriers, including interpreters, transportation, and prescription assistance
  • Serve as system navigator and point of contact for patients and families
  • Advocate with carriers for timely approval of necessary patient supplies and services
  • Identify and use cultural and community resources
  • Establish and maintain relationships with service providers
  • Ensure timely referral processing and remind patients of appointments by mail or phone
  • Keep primary care charts updated with specialist consults, hospitalizations, ER visits, and relevant community organizations
  • Follow the Code of Conduct and HIPAA guidelines

Requirements

What you’ll need
  • High school diploma, sometimes combined with medical assistant certification
  • Strong customer service focus
  • Effective verbal and written communication skills
  • Teamwork orientation
  • Organized and able to manage competing priorities
  • Good judgment
  • Resourcefulness in problem solving
  • Able to take and follow through with delegated tasks and accountability
  • Bilingual skills preferred (English/Spanish)
