About the role
Key responsibilities & impact- Optimize and clarify the medical care needs and positive functional capacity profiles of rehabilitation patients
- Provide career counseling and job placement support for rehabilitation patients
- Conduct profiling, application training, and job-market research through individualized educational support, with appropriate written documentation
- Serve as a person-centered navigator to systematically coordinate care and support services
- Cooperate on a case-by-case basis within a network of funding agencies and service providers, including rehabilitation facilities, social benefit providers, counseling centers, and self-help groups
- Work in accordance with the German Pension Insurance (DRV) specialist concept “Case Management Following Medical Rehabilitation”
Requirements
What you’ll need- Completed degree in a human services discipline—social work, education, or psychology—and at least two years of professional experience in counseling related to career (re)orientation
- A completed degree in psychology additionally requires proof of at least 40 instructional units of knowledge in social law
- Alternatively: vocational training in the health and social services sector or further training in rehabilitation, together with at least five years of professional experience in counseling related to career (re)orientation
- Proof of methodological and social-law competencies: 160 instructional units in counseling skills and 40 instructional units in social law
- Customer orientation, ability to motivate, empathy, and persuasiveness
- Further training in Case Management or CDMP certification is desirable
- Knowledge of the structures of social benefit providers and rehabilitation law
- Experience in vocational and medical rehabilitation, as well as solution-focused case management
- Good knowledge of MS Office programs, particularly Microsoft Word, and internet applications
- Mobility within the local area; use of a personal vehicle is preferred
