About the role
Key responsibilities & impact- Complete Carelon authorizations and ensure follow-up for approval
- Plan and facilitate educational activity-based groups
- Complete initial intakes and screenings in the office and within the community
- Develop Individual Psychiatric Rehabilitation Treatment plans
- Provide care coordination, develop age-appropriate daily living skills, increase community integration, and help clients meet rehabilitation goals
- Track and organize documentation and authorization due dates
- Communicate with internal and external program staff
- Develop and maintain relationships with other providers in the community
- Conduct internal and external chart reviews and develop improvement plans as needed
- Review and sign off on PRP Counselor clinical documentation
- Attend and lead meetings with client treatment teams
- Assist in training and supervision of direct service staff
- Complete Electronic Health Record documentation, including daily contact notes, assessments, and monthly summaries
- Perform case manager duties for designated clients or on an interim basis
- Attend annual cultural competency and responsiveness trainings
- Perform other duties as requested by the supervisor
Requirements
What you’ll need- Full-time position reporting to the Program Director
- Minimum 2 years of relevant supervisory experience working with adults who have a mental illness diagnosis
- Minimum 3 years’ experience working with adults with an SMI diagnosis (serious mental illness)
- Knowledge of COMAR regulations for PRP is mandatory
- Valid professional license: LMSW, LGPC, LCSW, LCSW-C, LCPC, LCMFT, or CPRP
- Must pass state background check
- Work in the office at least 3 full days per week; the other 2 days may be worked remotely
- Must be able to work in person at all locations whenever needed
- Must be a Maryland-licensed mental health professional or have CPRP
- At least 2 years of experience working with clients with severe emotional and/or psychiatric problems
- Ability to use a computer, phone, and electronic health record
- Maryland State driver's license, reliable vehicle, and proof of insurance
- Comfortable meeting clients in their home and community
- Willingness to travel between program sites, client homes, and community centers as needed
