About the role

Key responsibilities & impact
  • Create, plan, and organize teams defined by families
  • Develop, lead, and facilitate child and family team meetings
  • Provide care coordination in families’ homes, communities, or preferred locations
  • Partner with youth, families, natural supports, and service providers to identify care coordination needs and services
  • Lead teams in identifying needs, goals, and action steps
  • Ensure youth and families have voice and choice in coordinated care and services
  • Maintain documentation required to access and maintain OhioRISE benefits
  • Meet ISBH OhioRISE quality and fidelity requirements
  • Maintain knowledge of community resources
  • Attend mandatory in-person and virtual trainings and supervision, requiring travel
  • Complete assigned trainings and maintain required certifications
  • Follow confidentiality, professionalism, incident-reporting, HIPAA, mandated-reporting, and ethical requirements
  • Use databases and Electronic Health Record systems for accurate client documentation
  • Travel within the assigned service area
  • Perform other duties as assigned

Requirements

What you’ll need
  • Three years of experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred; certification provided at time of employment
  • Two years of experience in a coordinated supportive services or care coordination role preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred
  • Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development required
  • Qualifications and experience needed to work with children and families experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders, and engagement with child-serving systems required
  • Effective verbal and written communication required
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings required
  • Ability to maintain confidential information required
  • Ability to adhere to safety rules, regulations, and requirements required
  • Ability to bill Medicaid without restrictions required
  • Valid driver's license and ability to operate a motor vehicle required
  • Appropriate auto insurance coverage required
  • Ability to manage deadlines in a fast-paced, high-volume environment required
  • Ability to operate in an Internet-based, automated office environment required
  • Ability to conduct and participate in on-camera video calls and meetings required
  • Minimum 100 Mbps high-speed internet connection required
  • Prolonged periods of sitting at a desk and working on a computer required
  • Ability to lift up to 15 pounds at times required
