About the role
Key responsibilities & impact- Create, plan, and organize teams as defined by families
- Develop, lead, and facilitate child and family team meetings
- Provide care coordination in families' homes, communities, or preferred locations
- Partner with youth, families, natural supports, and service providers to identify care coordination needs and services
- Support youth at home, in school, and in their communities
- Lead teams in identifying needs, goals, and action steps
- Ensure youth and families have voice and choice in coordinated care and services
- Maintain documentation required to access and maintain OhioRISE benefits
- Meet ISBH OhioRISE quality and fidelity requirements
- Maintain knowledge of community resources
- Attend required trainings and supervision, including travel for in-person activities
- Complete assigned trainings and maintain required certifications
- Follow ISBH confidentiality, professionalism, and incident-reporting policies
- Use databases and Electronic Health Record systems for client documentation
- Maintain HIPAA compliance, mandated reporting, and professional ethical guidelines
- Travel within the assigned service area
- Perform other duties as assigned
Requirements
What you’ll need- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
- Knowledge and experience in Hi-Fidelity Wraparound preferred; certification provided at time of employment
- Two years of experience in a coordinated supportive services or care coordination role preferred
- Experience working with people with autism spectrum disorders and developmental disabilities preferred
- Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development required
- Qualifications and experience working with children and families experiencing serious emotional disturbance, trauma, co-occurring behavioral health disorders, and engagement with child-serving systems required
- Effective verbal and written communication required
- Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings required
- Ability to maintain confidential information required
- Ability to adhere to safety rules, regulations, and requirements required
- Ability to bill Medicaid without restrictions required
- Valid driver's license and ability to operate a motor vehicle required
- Appropriate auto insurance coverage required
- Ability to manage deadlines in a fast-paced, high-volume environment required
- Ability to operate in an Internet-based, automated office environment required
- Ability to conduct and receive on-camera video calls and/or meetings required
- Minimum 100 Mbps high-speed internet connection required
- Prolonged periods of sitting at a desk and working on a computer required
- Ability to lift up to 15 pounds at times required
