About the role
Key responsibilities & impact- Assist with immediate care coordination needs of youth hospitalized due to a behavioral health condition and enrolled in the OhioRISE program
- Support families with the hospital discharge process
- Educate families and community partners about the OhioRISE program
- Make referrals for follow-up care and services
- Attend in-person and virtual meetings to complete engagement milestones
- Develop crisis safety plans, assessments, and care plans
- Schedule and facilitate Child and Family team meetings
- Coordinate family-based services in homes, schools, and communities
- Identify family strengths, cultural factors, care coordination needs, and appropriate services
- Build positive relationships with children, youth, families, and child and family teams
- Develop partnerships with community resources
- Maintain documentation and participate in program evaluation, team and program planning meetings, and cross-systems training
- Acquire knowledge of community resources and maintain required training, including High Fidelity Wraparound, MI, and Cultural Humility
- Perform other duties as assigned
Requirements
What you’ll need- Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
- Knowledge and experience in Hi-Fidelity Wraparound preferred; certification provided at time of employment
- Two years of experience in a coordinated supportive services or care coordination role preferred
- Experience working with people with autism spectrum disorders and developmental disabilities preferred
- Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
- Cultural humility or responsiveness, with training and experience to manage complex cases
- Qualifications and experience working with children and families experiencing serious emotional disturbance, trauma, co-occurring behavioral health disorders, and engagement with child-serving systems
- Excellent organizational skills and ability to prioritize multiple tasks
- Demonstrated high degree of cultural awareness
- Experience with multi-need individuals and families
- Broad knowledge of community service systems
- Willingness to participate in and lead cross-systems care coordination
- Effective verbal and written communication
- Ability to operate in an Internet-based, automated office environment
- Valid Driver License required
- Ability to receive and conduct video calls and/or meetings in an automated office environment
- Minimum 100 Mbps high-speed internet connection required
