About the role

Key responsibilities & impact
  • Assist with immediate care coordination needs of youth hospitalized due to behavioral health conditions and enrolled in the OhioRISE program
  • Support families through the hospital discharge process
  • Educate families and community partners about the OhioRISE program
  • Make referrals for follow-up care and services
  • Attend in-person and virtual meetings to complete engagement milestones
  • Develop crisis safety plans, assessments, and care plans
  • Schedule and facilitate Child and Family team meetings until warm handoff to the youth’s ongoing care coordinator
  • Identify care coordination needs and services with families
  • Define cultural factors influencing strengths, functioning, and family interaction styles
  • Identify strengths of children, youth, and families to support engagement and healthy outcomes
  • Coordinate family-based services in homes, schools, and communities
  • Ensure care-plan services are appropriate, least restrictive, and meet safety and treatment needs
  • Build positive relationships with children, youth, families, and child and family teams
  • Develop partnerships with community resources
  • Maintain documentation and participate in program evaluation
  • Attend team and program planning meetings and cross-systems training
  • Maintain knowledge of community resources
  • Perform other duties as assigned

Requirements

What you’ll need
  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field: three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher
  • Knowledge and experience in Hi-Fidelity Wraparound preferred; certification provided at time of employment
  • Two years of experience in a coordinated supportive services or care coordination role preferred
  • Experience working with people with autism spectrum disorders and developmental disabilities preferred
  • Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
  • Cultural humility or responsiveness, with training and experience to manage complex cases
  • Qualifications and experience working with children and families experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders, and engagement with child-serving systems
  • Excellent organizational skills and ability to prioritize multiple tasks
  • High degree of cultural awareness
  • Experience with multi-need individuals and families
  • Broad knowledge of community service systems
  • Willingness to participate in and lead cross-systems care coordination
  • Effective verbal and written communication
  • Ability to operate in an Internet-based, automated office environment
  • Valid driver's license required
  • Ability to conduct and receive video calls and/or meetings in an automated office environment
  • Ability to maintain a minimum 100 Mbps high-speed internet connection
  • Ongoing training requirements including High Fidelity Wraparound, MI, and Cultural Humility
