About the role
Key responsibilities & impact- Conduct initial and ongoing assessments to identify patient status and individual needs
- Promote client self-determination
- Identify appropriate providers and facilities across the continuum of services
- Use resources and case management interventions to facilitate case management plans
- Develop case management plans with consumers and healthcare team members
- Identify goals, interventions, resources, reevaluation timeframes, and collaborative approaches
- Execute and document case management activities and interventions
- Gather information and documentation to determine plan effectiveness
- Evaluate and document plan effectiveness and modify plans as appropriate
- Measure and modify interventions to determine outcomes
- Facilitate authorizations and referrals within benefit structures, contracts, and regulations
- Document contacts according to best practices and required templates
- Notify claims personnel about anticipated surgeries, benefit exceptions, and significant events
- Proactively manage disability durations using evidence-based criteria
- Collaborate with patients/injured workers, providers, clients, employers, and internal team members
- Perform triage intake and coordinate care according to client instructions
- Administer leave and manage short-term disability according to regulations, policies, and plan documents
- Primarily support short-term disability and workers’ compensation, with potential group health benefits case management
Requirements
What you’ll need- Current registered nurse (RN) license
- 1+ years of case management experience with a return-to-work focus
- 2 years of clinical experience
- Two years full-time equivalent of direct clinical care to the consumer
- Licensure or certification in a health or human services discipline allowing independent assessment within scope of practice
- At least one of: URAC-approved case manager certification; bachelor’s or higher degree in a health or human services-related field; or current RN license
- Excellent written and verbal communication skills
- Proven customer service skills
- Effective time management skills
- Computer proficiency, especially Microsoft Office Suite, Outlook, Word, and Excel
- Willingness to obtain advanced certification when eligible
- Willingness to obtain additional state licensure as required by business needs
- Occasional travel to clients, the St. Charles, IL headquarters, and business partners
- Valid driver’s license required
- Availability during core hours of 8:30 a.m. to 5:00 p.m., Monday through Friday
- Successful substance, credit, and criminal background screenings
- Successful computer literacy testing
