About the role

Key responsibilities & impact
  • Support practices with clinical tools and resources to manage patients with long-term conditions
  • Ensure clinical services are delivered within protocols, service operating instructions, and systems
  • Identify opportunities for clinical reviews at practice, PCN, or ICB level where there is clinical need
  • Enhance awareness of Interface Clinical Services within primary care
  • Develop long-lasting relationships with practices and local NHS organisations
  • Support patients with chronic conditions in primary care settings

Requirements

What you’ll need
  • Must be a GPhC registered pharmacist
  • Minimum of 2 years’ post-qualification experience (PQE)
  • Primary care experience desirable
  • PCPEP qualification desirable
  • Full UK driving licence
  • Willing to travel as required and work remotely from home
  • Passion for making a positive difference to patients’ long-term health
  • Driven work ethic
  • Sponsorship is not available for this opportunity
