About the role

Key responsibilities & impact
  • Ensure the team provides initial coordination of information with clients, family members, physicians, referral sources, facility staff, agency staff, and others involved in continuity of client care
  • Inform referral sources, clients, family members, and physicians about response time and follow-up plans
  • Inform clinical office staff of potential admissions
  • Accurately document information in the client’s medical record
  • Input patient information into the EMR and update it with the correct level of care
  • Complete and copy necessary paperwork to prepare patient files for admission
  • Complete referral intake forms, copy referral information, attach it for clinical office staff review, and scan/upload referral information for admissions clinicians
  • Establish, maintain, and close patient EMR and paper charts according to regulatory and office guidelines
  • Mail, fax, or email required documentation as needed
  • Verify insurance coverage and collaborate to procure needed information
  • Create morning and evening census reports

Requirements

What you’ll need
  • Associates degree is preferred and equivalent with successfully completed course work or equivalent experience in medical terminology and/or working in healthcare environments
  • Minimum of two years’ experience working in hospice REQUIRED
  • Experience in dealing with a variety of payers in healthcare, e.g., Medicare, Medicaid, and commercial payors, preferred
  • Customer service focused, including the ability to set the caller at ease and encourage open dialogue about the client’s needs
  • Ability to work with a variety of team members representing other functional areas that directly or indirectly impact client intake/admission
  • Ability to maintain composure in addressing stressful and/or challenging situations
  • Prior use of Electronic Medical Records (EMR), preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
  • Monday-Friday 3PM-11PM schedule
  • Monday-Friday 11PM-8AM on-call expectations plus one shift every 12th weekend
  • Territory: Barrington, Deerfield, Crystal Lake, or Palos Heights
