About the role
Key responsibilities & impact- Conduct professional, objective assessments of older adult clients and their families, including psychosocial histories, in-person meetings, and communication with collaterals
- Develop effective relationships with clients, families, responsible parties, and care providers across care settings
- Create and communicate client-centered recommendations for care, resources, education, and support
- Assist with implementing and overseeing recommendations, including home care, assisted living, and nursing home arrangements
- Provide client-centered problem-solving, ongoing support, and advocacy
- Arrange and/or accompany clients to medical appointments and arrange transportation when needed
- Provide feedback to families and ensure medical orders are followed through
- Support and educate families and loved ones coping with loss, dementia, illness, change, and resistance
- Document billing and interventions daily
- Attend weekly in-person supervision with the Director of Clinical Services
- Attend monthly group supervision, staff meetings, and facility tours
- Complete professional development to obtain and maintain licensure and certification
- Apply for and renew licensure and certification as required by governing bodies
- Abide by the Standards of Practice for Aging Life Care Professionals as determined by ALCA
Requirements
What you’ll need- Advanced degree preferred; licensure required in social work, nursing, geriatrics, mental health, or a related field
- 3–5 years’ experience working with older adults and families
- Excellent clinical skills
- Knowledge of family dynamics and family systems
- Highly organized and able to manage a fee-for-service caseload effectively
- Ability to work in a non-traditional, highly professional work environment
- Outstanding interpersonal and communication skills
- Knowledge of and comfort using electronics, including a smartphone and laptop/desktop computer
- Ability to drive with access to a vehicle
- Obtain and/or maintain membership in the Aging Life Care Association (ALCA)
- Obtain and maintain certification as required by ALCA
- Maintain professional licensure as required by the Commonwealth of Massachusetts
