About the role

Key responsibilities & impact
  • Develop individualized care plans with prioritized goals based on patient and caregiver preferences
  • Make outbound calls to assess patients’ current health status
  • Identify gaps or barriers in treatment plans
  • Coordinate member care and make referrals to outside sources
  • Ensure patients discharging from hospitals or emergency departments receive necessary services and resources, including medication reconciliation
  • Triage symptom-based calls using approved protocols
  • Enroll patients and manage alerts for home monitoring programs
  • Provide approved general health information and guidance to public callers
  • Document workflows in EPIC for tracking and quality assessment
  • Provide patient education for wellness and disease self-management
  • Educate members on chronic and acute disease processes
  • Collaborate with care partners, leadership, and physicians on clinical questions, strategies, and care plans
  • Support quality and cost management objectives

Requirements

What you’ll need
  • Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing
  • 2 years of professional nursing or care management experience
  • Proof of citizenship or immigration status to verify lawful right to work in the United States
