Provide rehabilitative services to residents under specific medical orders as part of a coordinated team
Plan initial and subsequent treatment programs based on clinical findings
Participate in clinical conferences to share and coordinate patients’ plans of care
Provide individualized, one-on-one treatment sessions with a functional approach
Collaborate with residents, families, and the interdisciplinary team to support meaningful outcomes
Serve residents across Independent Living, Assisted Living, and Memory Care
Provide care throughout the senior living community, including spaces where residents live and move

What you’ll need

Degree in Physical Therapy
Current Physical Therapy license in the state of Illinois, or ability to obtain prior to beginning employment
Excellent interpersonal, communication, clinical, and collaborative skills
Commitment to providing individualized, resident-centered care
70% productivity expectation
Full-time salaried position with 40 guaranteed hours per week
