About the role

Key responsibilities & impact

Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members
Review medications and medical histories
Educate members about their health and answer questions
Help close gaps in care
Provide connections to appropriate care for members’ unique needs
Work in a private space with adequate internet connection
Participate in regular manager, team, and clinical meetings
Meet Signify Health operational, clinical, and member experience KPIs and metrics
Deliver Video Visits/telehealth services only
Maintain training, affiliation, and compliance requirements

Requirements

What you’ll need

Board-certified nurse practitioner or physician assistant
Active state licenses in coverage areas
Ability to obtain and maintain multi-state licenses in coverage area, as required
FNP or AGNP certification preferred for NPs
Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or similar format preferred
Provide 40 hours of availability weekly
Consistently provide 320+ capacity units per month
Consistently pace 220+ completed visits per month
Maintain training, affiliation, and compliance with Video Visit requirements
Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Maintain board certification
Meet remote visit delivery requirements, including broadband connectivity with video and audio enabled
Quiet setting with a neutral background for conducting visits
Ability to uphold health plan member privacy per CVS Health guidelines
Effective verbal, written, and electronic communication skills
Ability to work without direct supervision and practice autonomously
Proficiency with information management and technology
Ability to interact remotely with members, sit, write, operate computer and phone, speak intelligibly, and hear member responses
Ability to view and read computer screens, iPad, and other electronic devices
