About the role

Key responsibilities & impact

Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members
Review medications and medical histories
Educate members about their health and answer their questions
Help close gaps in care and connect members to appropriate care
Work in a private space with adequate internet connection
Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Engage regularly in manager, team, and clinical meetings
Meet Signify Health operational, clinical, and member experience KPIs and metrics
Provide 40 hours of availability weekly
Maintain 320+ capacity units and 220+ completed visits per month
Conduct Video Visits through telehealth services only

Requirements

What you’ll need

Full-time, board-certified nurse practitioner or physician assistant
Active state licenses in coverage areas
FNP or AGNP certification preferred for nurse practitioners
Maintain board certification
Able to obtain and maintain multi-state licenses in coverage area, as required
Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred
Availability for 40 hours weekly
Consistently provide 320+ capacity units per month
Consistently pace 220+ completed visits per month
Maintain training, affiliation, and compliance with Video Visit requirements
Meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled
Quiet setting with a neutral background for conducting visits
Ability to uphold health plan member privacy per CVS Health guidelines
Effective verbal, written, and electronic communication skills
Ability to work without direct supervision and practice autonomously
Proficiency with information management and technology
