About the role

Key responsibilities & impact

Conduct approximately 30-minute online Video Visits with Medicare Advantage, Medicaid, and certain commercial plan members
Review medications and medical histories during Video Visits
Educate members about their health and answer their questions
Help close gaps in care
Work in a private space with adequate internet connection
Provide 40 hours of weekly availability and 320+ capacity units per month
Maintain a strong visit completion rate of 220+ completed visits per month
Maintain training, affiliation, and compliance with Video Visit requirements
Attend regular manager, team, and clinical meetings
Meet Signify Health operational, clinical, and member experience KPIs and metrics
Deliver telehealth Video Visits only
Practice autonomously while maintaining member privacy and required remote-visit standards

Requirements

What you’ll need

Nurse Practitioner or Physician Assistant; the posting specifically describes hiring full-time, board-certified nurse practitioners
Board certification required
Active state licenses in coverage areas required
Must be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Ability to obtain and maintain multi-state licenses in coverage area, as required
FNP or AGNP certification preferred for NPs
Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred
Provide 40 hours of availability weekly
Ability to conduct visits from a private, quiet space with a neutral background and adequate broadband connectivity with video and audio enabled
Ability to uphold health plan member privacy per CVS Health guidelines
Effective verbal, written, and electronic communication skills
Ability to work without direct supervision and practice autonomously
Proficiency with information management and technology
Ability to interact with members remotely, sit, write, operate computers and phones, speak intelligibly, and hear member responses
Ability to view and read computer screens, iPads, and other electronic devices
