About the role
Key responsibilities & impact- Develop supportive relationships with older adults and people with disabilities
- Foster consumer independence and serve as a central source of support and resources
- Disseminate information and make appropriate referrals
- Create, implement, and update service plans for consumers in collaboration with formal and informal support systems
- Conduct virtual and in-person comprehensive, person-centered assessments with consumers, care partners, and health-care professionals
- Collaborate with internal and external providers to coordinate service plans through meetings and telephone contacts
- Maintain documentation and consumer records according to regulations
- Assist consumers, families, caregivers, and informal supports with problem-solving
- Facilitate team meetings and case/family conferences as needed
- Maintain current knowledge of services available to older adults and people with disabilities in the service area
- Advocate for consumers to obtain needed services
- Participate in interdisciplinary team meetings, staff meetings, and trainings
- Represent LifePath when directed
- Provide department coverage as assigned
- Assist in training new staff through shadowing visits and sharing information and tips
- Perform other duties assigned by supervisory or management staff
- Report to the Information and Caregiver Resource Center Program Director
- Split work between the office and travel to meetings, trainings, and consumers’ homes
Requirements
What you’ll need- 2-3 years of previous experience in health or human services required
- Bachelor’s Degree preferred
- License in Social Work is a plus
- Willingness and sensitivity for working with older adults
- Strong documentation skills
- Familiarity with electronic health records (EHR) or care management platforms
- Familiarity with Microsoft programs
- Ability to work independently and as part of an interdisciplinary team
- Exceptional interpersonal and communication skills
- Capacity to complete comprehensive needs assessments using observation, listening, and interviewing skills
- Ability to solve problems and find solutions
- Flexibility to meet multiple and changing program demands
- Familiarity with or willingness to learn about health and human services programs
- Demonstrates cultural humility and cultural responsiveness
- Capacity to see computer screens and read written material
- Capacity to drive a car and navigate uneven terrain by foot in all weather conditions
- Capacity to hear and speak on the telephone
- Capacity for fine manipulation when using office equipment
- Ability to climb stairs
- Ability to sit or stand for extended periods
- Occasional capacity to lift up to 25 pounds
- Ability to understand and/or interpret complicated program instructions, regulations, and laws
- Capacity to learn complicated computer software
