About the role
Key responsibilities & impact- Provide spiritual assessments and assistance to patients and their families
- Assure spiritual assessment of patients and families/caregivers and render appropriate services in a timely manner
- Facilitate individualized plans of care by participating in comprehensive assessments
- Serve as liaison and support to community chaplains and spiritual counselors
- Maintain records of spiritual care service utilization and related activities
- Document direct services and ongoing communication with community chaplains and spiritual counselors
- Attend patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary group
- Cover primarily Bolingbrook, Romeoville, Homer Glen and surrounding areas
Requirements
What you’ll need- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
- Clinical pastoral education certification required
- Minimum of one year’s experience in health care
- Palliative or hospice experience preferred
- Must be a licensed driver
- Must have an automobile insured in accordance with state or organization requirements
- Automobile must be in good working order
- Full-time 40 hours with requirement of 4-5 on call shifts per month
