About the role
Key responsibilities & impact- Oversee an interdisciplinary team including Registered Nurse Case Managers (RNCMs), social workers, and chaplains
- Direct and deliver high-quality hospice care within home and community settings
- Enhance communication and support RN Case Managers, Social Workers, and Chaplains in the field
- Conduct weekly IDT meetings for the designated team
- Guide new staff through employee orientation and hospice-care education
- Conduct quarterly performance check-ins with the team
- Participate in quarterly and as-needed ride-along visits with the team
- Maintain a small caseload
- Complete initial, comprehensive, and ongoing patient and family assessments to determine hospice needs
- Participate in on-call coverage
Requirements
What you’ll need- Current RN license
- Minimum of 3 years of hospice with supervisory experience
- Valid driver's license and insured, reliable vehicle
