About the role
Key responsibilities & impact- Provide hospice care through evaluation and assessment of health histories
- Promote health and diagnose medical conditions
- Manage health problems by directing and developing individualized plans of care
- Prescribe medications and treatments
- Coordinate and collaborate with patients, families and other healthcare practitioners
- Conduct face-to-face visits
- Sign E-scripts
- Attend IDT meetings
- Assist team members with patient and family needs
- Travel to patients’ homes, nursing homes or long-term care facilities
- Manage health conditions and terminal illnesses through consultations, referrals and coordination of hospice services
- Collaborate with interdisciplinary team members to control patient symptoms
- Review clinical information and coordinate and oversee patient care
- Complete hospice face-to-face visit requirements before the third and subsequent hospice benefit periods
- Assess hospice eligibility according to conditions of participation and local coverage determinations
- Provide clinical information to the hospice medical director for hospice recertification eligibility
Requirements
What you’ll need- Graduate from an accredited School of Nursing
- Graduate from an accredited Nurse Practitioner Program
- Minimum of one (1) year Nurse Practitioner experience
- Current nursing licensure in State
- CPR certification
- Certification in a specialist area preferred, e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics
- Ability to conduct face-to-face visits and travel to patients’ homes, nursing homes or long-term care facilities
- Availability to work typically Monday–Friday, 8:00 a.m.–4:30 p.m.
