About the role
Key responsibilities & impact- Deliver skilled nursing care and hospice services in home and community settings
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments while monitoring patient symptoms and outcomes
- Educate patients and caregivers on care, self-management, and disease progression
- Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
- Maintain accurate documentation
- Participate in on-call coverage as needed
- Plan and direct services to meet the needs of individuals and families/caregivers within their homes and communities
Requirements
What you’ll need- Current RN license
- Graduate of accredited nursing program
- Minimum of 2 years of nursing experience
- At least 1 year in public health, home care, or hospice preferred
- Valid driver’s license
- Insured, reliable vehicle
- Minimum of 20 on-call hours per month, which may increase
- Coverage of Southern Cook County, including Palos Park, Crestwood, Hickory Hills, and surrounding areas
