About the role

Key responsibilities & impact
  • Deliver skilled nursing care and hospice services in home and community settings
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer medications and treatments while monitoring patient symptoms and outcomes
  • Educate patients and caregivers on care, self-management, and disease progression
  • Collaborate with the interdisciplinary team to ensure coordinated, high-quality care

  • Participate in on-call coverage as needed
  • Plan and direct services to meet the needs of individuals and families/caregivers within their homes and communities

Requirements

What you’ll need
  • Current RN license
  • Graduate of accredited nursing program
  • Minimum of 2 years of nursing experience
  • At least 1 year in public health, home care, or hospice preferred
  • Valid driver’s license
  • Insured, reliable vehicle
  • Monday–Friday schedule
  • Minimum of 20 on-call hours per month, which may increase
  • Coverage may include a portion of Will and Southern Cook Counties (Orland Park, Homer Glen, Lockport & surrounding areas)
