About the role

Key responsibilities & impact
  • Collaborates with Practice Leadership to conduct Daily Huddles and Weekly Care Team meetings
  • Reviews practice population health data to improve care and achieve practice goals
  • Prepares patients for exams by obtaining and documenting vital signs and chief complaints
  • Prepares exam rooms based on patient complaints
  • Administers medications, applies appliances and dressings, and performs first aid as ordered
  • Performs phlebotomy and obtains specimens for processing
  • Administers vaccines and completes required documentation
  • Performs diagnostic tests such as EKG and Pulmonary Function tests
  • Supports Population Health efforts through panel management, test and referral tracking, and care coordination
  • Leads patient education and supports/trains Medical Assistants
  • Leads self-management efforts for patients with chronic conditions, including health coaching and motivational interviewing support
  • Coordinates transitions of care, including post-discharge and post-ED outreach and follow-up scheduling
  • Facilitates hospital and other records into the EMR before visits
  • Manages Coumadin follow-up
  • Maintains clinical supply par levels
  • Performs and documents daily regulatory testing of clinical equipment

Requirements

What you’ll need
  • Graduate of approved LPN School of Nursing
  • Current NJ LPN License
  • Current CPR certification
  • Minimum 6 months experience in a physician practice
  • Knowledge of minor surgical procedures and sterile technique
  • Experience in primary care preferred
