About the role

Key responsibilities & impact
  • Serve as the first clinical connection for patients and families beginning hospice services
  • Conduct assessments and coordinate care in patients' homes or other care facilities
  • Collaborate with external medical professionals and referral sources to organize and complete hospice admissions
  • Build relationships with referral partners to streamline admissions and increase awareness of services
  • Coordinate with transport companies and durable medical equipment providers
  • Complete initial assessments and plan-of-care documentation during admission visits
  • Document hospice eligibility based on clinical criteria, including uncontrolled pain, advanced disease symptoms, and declining condition
  • Educate and support caregivers and family members about end-of-life expectations, medications, symptom management, and changes in condition
  • Complete real-time documentation for accuracy and compliance
  • Coordinate care with the Interdisciplinary Team, including physicians, nurse practitioners, RNs, social workers, chaplains, and CNAs
  • Communicate and carry out admission orders from nurse practitioners and physicians

Requirements

What you’ll need
  • Graduate of an accredited nursing program (AD, Diploma, or BSN)
  • Current RN license in North Carolina
  • At least 2 years of nursing experience
  • Hospice or home care experience preferred
  • Strong communication and documentation skills
  • Comfortable spending long amounts of time in your car and driving frequently
  • Ability to work a schedule operating 7 days per week
  • Ability to work four 10-hour shifts per week, from 12pm to 10pm
  • Ability to work rotating weekends and holidays
