About the role

Key responsibilities & impact
  • Provide high-quality care management and acute care to patients enrolled in the Primarily Home program
  • Conduct in-home comprehensive assessments
  • Develop individualized care plans tailored to each patient’s needs
  • Assess patients’ clinical and environmental needs in the home
  • Foster an overall healing environment
  • Educate and provide ongoing compassionate and empathetic support to patients and their families
  • Ensure patients and families have the resources and understanding needed to manage care at home
  • Collaborate with interdisciplinary teams to coordinate care and optimize patient outcomes
  • Drive to patients’ homes and perform physical activities including patient transfers and carrying medical supplies

Requirements

What you’ll need
  • Associate degree combined with 3+ years of relevant experience or the knowledge, skill, or ability to succeed in the role
  • Registered nurse in good standing in the state of Missouri and/or Illinois and eligibility for licensure in other states
  • Must have or obtain Basic Life Support (BLS) within 30 days of hire
  • Valid Driver’s license, automobile insurance, and access to reliable transportation to visit patients
  • Authorized to work in the United States
  • Acute and home care experience a plus
  • Bachelor's degree from an accredited program with a major in nursing preferred
  • Ability to drive a vehicle, transfer patients, lift or carry medical supplies, operate complex and delicate equipment, operate a personal computer, and frequently interact with care providers and patients
  • Required to complete a pre-employment drug screen
  • Member-facing personnel required to have completed, at minimum, the initial series of an approved COVID-19 vaccine
