Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered
Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents
Create a service plan in conjunction with the client to develop goals and objectives consistent with what the client wishes to accomplish
Counsel, advocate, and teach clients the skills identified in their service plan face to face in home or virtually
Respond to client crisis situations within program parameters in a timely manner according to program protocols

Requirements

What you’ll need

Bachelor's or Master's Degree in Social Work, Counseling, Psychology, Criminal Justice, or other Human Behavior Science
Must be a licensed driver with access to dependable personal transportation and auto insurance
Preferred 2 years experience with Child Welfare, Adoption, Foster Care, Community Resources or Care Coordinator
