About the role

Key responsibilities & impact
  • Provide comprehensive, high-quality primary care through a variable mix of in-person and virtual care options
  • Support the patient panel of 1–3 designated providers in a specific primary care site or Pod
  • Collaborate with in-clinic primary care providers to deliver evidence-based virtual primary care
  • Conduct virtual histories and symptom assessments and develop evidence-based treatment and follow-up plans
  • Perform urgent care and medical triage visits in person or virtually as needed
  • Support the Transitions of Care nurse team with urgent patient care and care coordination for patients transitioning from acute care settings to home
  • Provide patient education, counseling, and follow-up care for wellness and chronic disease management
  • Assist PCPs with EHR inbox management by contacting patients about abnormal lab, radiology, or other results and developing management and follow-up plans
  • Complete EHR documentation within MCC timeliness standards
  • Update patient problem lists, manage referrals, and perform other asynchronous clinical care
  • Provide trauma-informed and culturally sensitive medical care

Requirements

What you’ll need
  • Advanced Practice Provider (NP/PA) from an accredited medical training program
  • Current California medical license and DEA certificate required
  • Minimum of 3 years of experience providing primary care in a family or community health setting
  • Experience with EHR systems; EPIC preferred
  • Prior experience working in a Federally Qualified Health Center (FQHC) a plus
  • Demonstrated ability to be proactive, flexible, and multitask in a busy clinical environment
  • Willingness to receive regular feedback and solve problems in real-time
  • Bilingual (Spanish/English) strongly desired
  • Excellent written and verbal communication skills
  • Commitment to diversity, equity, inclusion, and trauma-informed care principles
  • Active BLS Certification
  • Must maintain Continued Medical Education (CME) as required for certification renewals
  • Fulfill immunization and fit for duty regulatory requirements
  • Ability to navigate electronic medical records and related IT systems efficiently with training
  • Ability to perform confidential patient visits remotely with reliable, high-speed internet/telephonic services and a confidential remote workspace
  • Ability to tolerate prolonged standing and/or prolonged periods sitting at a desk and working on a computer
  • Must be available for on-site training as needed during onboarding or ongoing medical credentialing
