About the role
Key responsibilities & impact- Collaborate with all members of the care team to provide service to patients and staff
- Coordinate delivery of primary and specialty care services
- Perform independent in-person and telephone patient assessments
- Provide patient education and address patient concerns
- Coordinate with internal providers, external specialty providers, community health care resources, urgent care centers, and hospitals
- Prioritize and schedule specialty and primary care services
- Perform walk-in assessments and clinical triage using standard protocols
- Maintain medical supplies, instruments, and equipment
- Identify and facilitate closure of gaps in care
- Partner with providers and clinical support teams to improve quality outcomes and care navigation
- Review appointments and patient records for chronic disease care gaps
- Participate in daily huddles and team meetings
- Triage telephone calls and coordinate appropriate care and next steps
- Support health maintenance, wellness promotion, disease management, flu clinics, and community wellness initiatives
- Process prescription refills, DME orders, forms, reports, lab results, and telephone notes
- Develop, implement, and follow up on nursing plans of care
- Conduct nursing care visits, injections, nebulizer treatments, chronic disease visits, wound care, monitoring, testing, and counseling
- Perform point-of-care testing, including COVID, flu, strep, RSV, glucose, urinalysis, A1C, pregnancy, and drug-screen testing
- Utilize appropriate PPE per protocol
- Receive day-to-day clinical direction from on-site RN Clinical Leads, Clinical Quality Specialist, Clinical Supervisors, or Clinical Manager
Requirements
What you’ll need- Associate degree in nursing
- Minimum of 2 years’ experience
- At least one year as nurse responsible for independent decisions
- Current, unrestricted Registered Nurse licensure in Maine
- BLS required
- ACLS required for Cardiac Nurse
- Understanding of and alignment with Martin’s Point mission and values
- Flexibility, creativity, and excellent communication between many disciplines
- Ability to research and identify creative and effective solutions
- Ability to proactively seek clarification and solutions to process issues
- Complete and accurate documentation
- Ability to develop policies, procedures, and training on equipment
- Ability to act as a specialty resource to others
