About the role

Key responsibilities & impact
  • Conduct intake interviews with members and complete Social Determinants of Health assessments
  • Communicate the program’s purpose and potential impact on members’ well-being
  • Help patients identify socioeconomic issues affecting health and develop health/social management plans and goals
  • Partner with community organizations, governmental agencies, and local resources
  • Assist patients in understanding care plans and instructions
  • Help members access health-related services and overcome barriers to medical care and social services
  • Schedule appointments with social services agencies and assist with program applications
  • Provide support and advocacy during medical visits when necessary
  • Follow up with members and providers regarding health/social services
  • Maintain positive working relationships with members, providers, nurse case managers, agency representatives, and office staff
  • Work to reduce cultural and socioeconomic barriers between members and institutions
  • Maintain confidentiality of member records

Requirements

What you’ll need
  • High school diploma or GED
  • Valid Maryland State Driver’s License required
  • 2 years of healthcare experience, preferably in the MD/DC area with Medicaid members
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services
  • Knowledge of transportation and other barriers to care
  • Availability and willingness to travel 50–75% around Allegany and Garrett County, Maryland
  • Ability to work in clinical and community-based settings
  • Ability to observe confidentiality of member records in accordance with MPC policies and procedures
