About the role

Key responsibilities & impact

Provide medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP)
Conduct social service assessments and connect HIV-positive individuals with needed services offered by Federal, State, and Local community programs
Assess and identify patients’ individual needs and provide appropriate interventions
Provide advocacy, support, and education to achieve patient wellness
Develop and implement inclusive individualized care plans and make referrals as necessary
Assist clients in making informed decisions about their health and encourage health literacy
Ensure clients obtain certification/recertification for the Kentucky Ryan White Program
Work with patients and a multidisciplinary team to provide access to medication, treatment adherence counseling, and medical insurance
Provide appropriate documentation on multiple software platforms
Foster connections with clients, informal support, and community partners to ensure access to direct medical care
Strengthen professional practice by learning field best practices and developing new skills

Requirements

What you’ll need

A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred
Candidates with case management experience of two years or more will also be considered as viable applicants
Experience working in a nonprofit organization or a strong desire to work in a service-driven environment

Flexibility, as services are available to a large geographic area in Kentucky, which may require some travel
Valid driver's license with at least three years of driving experience and fewer than two moving violations in the past three years
Must be able to pass a drug screen and background test
