About the role
Key responsibilities & impact- Conduct comprehensive intake assessments to identify medical, behavioral, social, and environmental needs
- Develop individualized care plans supporting healthy pregnancies and positive birth outcomes
- Coordinate care with physicians, hospitals, specialists, behavioral health providers, and community agencies
- Conduct home visits, virtual visits, and telephone follow-ups
- Educate members on prenatal care, postpartum recovery, infant care, breastfeeding, safe sleep, nutrition, and maternal wellness
- Connect families with transportation, housing, food, childcare, behavioral health, and financial assistance resources
- Advocate for members to reduce healthcare barriers
- Monitor member progress and adjust care plans throughout pregnancy and postpartum
- Maintain accurate documentation in electronic health record systems and care management platforms
- Collaborate with interdisciplinary teams to promote coordinated, culturally responsive care
- Participate in quality improvement and community outreach activities
- Manage an assigned caseload of approximately 60 members
- Complete required outreach, home visits, telephonic contacts, documentation, quality assurance reviews, and audits
- Attend orientation, trainings, continuing education, team meetings, case conferences, and interdisciplinary collaboration
- Maintain compliance with state, federal, Medicaid, HIPAA, payer, regulatory, and organizational requirements
Requirements
What you’ll need- Bachelor's degree in Social Work, Public Health, Human Services, Nursing, Psychology, Sociology, Early Childhood Education, or a related field, OR minimum of two (2) years of experience in case management, care coordination, social services, healthcare, behavioral health, community health, or a related field
- Valid driver's license, reliable transportation, and automobile insurance
- Ability to travel throughout assigned service areas
- Strong organizational, communication, and documentation skills
- Ability to maintain confidentiality and comply with HIPAA requirements
- Preferred: Experience in maternal health, prenatal care, postpartum care, or women's health
- Preferred: Knowledge of Medicaid managed care or care coordination programs
- Preferred: Community Health Worker (CHW) experience
- Preferred: Experience serving high-risk or underserved populations
- Bilingual (English/Spanish) strongly preferred
