About the role
Key responsibilities & impact- Provide member-centered, evidence-based pregnancy case management across multiple products
- Conduct comprehensive assessments
- Develop, implement, and monitor individualized care plans
- Coordinate services across the healthcare continuum
- Deliver education and care coordination
- Identify gaps in care and advocate for maternal members
- Help reduce pregnancy- and birth-related complications and healthcare costs

Requirements
What you’ll need- Associate or bachelor’s degree in nursing
- 3+ years of clinical/acute care experience including direct maternal patient care
- Current, unrestricted RN license in the state of residence
- Certified Case Manager (CCM) preferred, or ability and commitment to obtain within two years of hire
- Experience working with the pregnancy population in a clinical or home care setting
- Experience working with multiple age groups and diverse ethnic and socioeconomic backgrounds
- Experience managing multiple computer systems and tools
- General working knowledge of the healthcare continuum
- Sound clinical foundation in maternal health, including pregnancy care and management of preeclampsia, gestational diabetes, and preterm labor
- Knowledge of postpartum recovery, including physical and mental health considerations
- Excellent verbal and written communication skills and ability to present in a group setting
- Ability to work efficiently toward department benchmarks
- Ability to work in a fast-paced, fluid, ever-changing environment and manage multiple complex issues
- Must be legally authorized to work in the United States at the time of application
