About the role
Key responsibilities & impact- Engage regularly with patients and track adherence to care plans
- Monitor patients' device readings and investigate out-of-bounds values
- Maintain detailed records of patient interactions and program details
- Stay current on regulations and compliance standards for CCM/RPM services
- Ensure patient enrollments comply with legal and ethical standards
- Monitor and track care performance against goals
- Support patients across multiple medical practices, workflows, teams, and patient populations
- Transition between practices according to operational needs while maintaining consistent care quality
- Create and track patients' care plans and accompany them throughout their healthcare journey
Requirements
What you’ll need- National Certification in Medical Assisting
- Residence in the US
- Strong written and verbal communication skills
- Empathy and the ability to connect with patients and their families
- Basic knowledge of chronic health conditions and their management
- Highly adaptable and comfortable working across different medical practices and teams
- Strong interpersonal and communication skills, with the ability to communicate clearly and naturally with colleagues
- Reliable, flexible, and comfortable working in a float pool environment
- Comfortable using technology for documentation and communication
- Strong attention to detail and organizational skills
- Ability to work independently and as part of a team
- National certification in Medical Assisting from AAMA, NHA, NCCT, NAHP, or similar is required; single-state certifications, diplomas, or completion certificates do not qualify
- Attach Medical Assistant certificate
- Monday-Friday availability
- Bilingual proficiency in English and an additional language is nice-to-have
