About the role
Key responsibilities & impact- Coordinate patient transfer logistics using client-defined workflows to support patient acceptance and bed placement
- Identify appropriate level of care, accepting provider, consultation, and access from hospitals, clinics, extended care facilities, and physician offices
- Ensure accurate clinical presentation, timely access, appropriate bed assignment, and accurate patient information documentation
- Collaborate with clinicians, physicians, hospital staff, medical staff, leadership teams, and outside agencies
- Use critical thinking and clinical decision-making to develop appropriate destination plans of care
- Act as a liaison between patients, physicians, hospital staff, client facilities, and outside agencies
- Monitor individual and team compliance and performance
- Support process changes and regulatory and quality initiatives
- Identify, trend, and report barriers to patient access and throughput
- Report safety, compliance, and operational risks through the defined chain of command
- Participate in process improvement, professional development, peer development, peer review, shared leadership, and quality data collection
- Maintain active RN licensure within the states where transfer center services are provided
- Perform other tasks and analysis as requested or directed
- Work assigned shifts on a six-week rotation, including every-other-weekend, on-call, and rotating holiday requirements
- Complete three weeks of orientation from 8:00 a.m. to 4:30 p.m. ET and align with a preceptor during the fourth week
Requirements
What you’ll need- Active Registered Nurse (RN) – Required
- Active multistate/compact registered nurse (RN) licensure – Required if applicable based on state of residence
- Associate Degree of Nursing or diploma (ADN) – Required
- 1 year of acute care experience – Required
- Must have a designated workspace with a locked door, per HIPAA regulations
- Secondary work location meeting HIPAA regulations is encouraged
- Minimum internet speed of 100 Mbps download and 20 Mbps upload at primary and secondary work locations
- Proficient in EHR data entry, telephony systems, and office systems
- Knowledge of EMTALA, Ethics and Compliance, quality initiatives, and HIPAA
- Microsoft Office tools, including Outlook, MS Teams, Excel, and Word
- Ability to review and interpret patient transfer case information
- Ability to escalate and inform regarding patient status changes
- Ability to record tasks and conversations regarding patient transfer
- Critical thinking, active listening, attention to detail, and multitasking
- Excellent oral and written communication skills
- Bachelor’s degree of nursing (BSN) – Preferred
- 3+ years of acute care experience – Preferred
- Healthcare contact center experience – Preferred
- Epic electronic health record experience – Preferred
- Genesys contact center experience – Preferred
