About the role

Key responsibilities & impact

Provide telephonic triage to determine the most appropriate level of care
Provide requested support and/or virtual monitoring
Offer recommendations, education and care advice using decision-making tools, clinical judgment and defined workflows
Participate in care coordination to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting
Schedule provider appointments and facilitate provider communication
Document accurately and promptly in the electronic medical record
Participate in virtual monitoring, reporting and escalation to support services
Support Conduit Health Partners business functions and continuity of operations
Participate in process improvement, professional development, peer development and peer review

Requirements

What you’ll need

RN license required in applicable state(s)
Multistate/Compact RN Licensure preferred
ADN or Diploma in Nursing required
BSN preferred
1 year of acute care nursing experience required
Triage experience preferred
EPIC Electronic Health Record preferred
Knowledge and skills to provide age-appropriate care across neonates, infants, pediatrics, adolescents, adults and geriatrics
Assessment of condition via telephone or video
Ability to inform medical professionals regarding patient conditions and care
Minimum internet speed of primary and secondary work locations: download 100 Mbps and upload 20 Mbps for the remote work environment
Ability to perform shift work
Ability to travel by car to off-site locations occasionally, including in adverse weather conditions
Manual dexterity, finger dexterity, color vision, far and near acuity, and ability to hear alarms, telephone and audio recordings
