About the role

Key responsibilities & impact
  • Conduct timely referral follow-up and intake scheduling
  • Provide new patient orientation and assessment
  • Develop and implement treatment plans and exercise prescriptions
  • Serve as case manager for a group of patients
  • Modify treatment plans according to changing patient needs
  • Perform biometric data, EKG, medical record/patient history, medication reconciliation, signs and symptoms, exercise tolerance, mental health, quality of life, and dietary habit assessments
  • Conduct submaximal and exercise tolerance testing, muscular strength and endurance testing, and flexibility testing
  • Communicate proper exercise execution, equipment use, and safety strategies
  • Identify educational needs and facilitate group education classes
  • Support patient goal setting, engagement, and behavior change using motivational interviewing and active listening
  • Document clinical status and exercise response in medical records
  • Report patient progress to referring providers and the medical director
  • Participate in infection control procedures and report equipment or environmental concerns
  • Contact patients regarding program participation and first-appointment scheduling

Requirements

What you’ll need
  • Bachelor's Degree in Exercise Physiology or related field required
  • If no Master's Degree, Certification through ACSM (Certified Clinical Exercise Physiologist) or AACVPR (Certified Cardiac Rehabilitation Professional or Pulmonary Rehabilitation Certificate) required within 3 years
  • Current BLS certification
  • Advanced Cardiac Life Support (ACLS) preferred
  • Interpretive/analytical skills
  • Effective interpersonal communication skills
  • Organizational skills
  • Windows software knowledge (Office)
