About the role
Key responsibilities & impact- Serve as a consistent care coordinator throughout the continuum of care for patients entering or within the health system
- Assess patients’ physical, psychological, and social needs
- Improve the care experience for patients and their families
- Provide nursing care in a clinical/office setting
- Coordinate communication with providers on behalf of patients
- Provide education, screening, prevention, and follow-up care
- Create seamless flow through the health system to facilitate patient care
- Develop and coordinate education, resources, and activities for patients and families
- Maintain reliable systems to document, track, and monitor patient data
- Perform clinical procedures within the scope of nursing practice and clinical training
- Perform telephone duties within the scope of the RN role
- Perform other assigned or requested job functions

Requirements
What you’ll need- Required - Associates Degree Nursing
- Required - Registered Nurse experience: 0-1 years
- Required - Basic Life Support certification from the American Heart Association
- Required - Registered Nurse license in Tennessee from the Tennessee Board of Nursing
- Preferred - Bachelor's Degree Nursing
- Preferred - Clinical nursing experience: 1-3 years
- Preferred - Certification in clinical specialty
- Knowledge of system health resources to navigate patients to appropriate care
- Excellent verbal and written communication skills
- Desire to problem solve and strong conflict resolution abilities
- Strong computer skills
- Customer service and patient care skills
- Ability to handle and administer medications, vaccinations, and immunizations as needed
- Ability to react quickly to emergency situations
- Ability to perform physical activities including climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion
- Good balance and coordination
- Ability to exert up to 50 lbs. of force occasionally and/or up to 25 lbs. frequently
- Close visual acuity, including color, depth perception, and field of vision
- Frequent invasive and non-invasive patient contact
- Ability to work with exposure to patient body fluids and hazardous or poisonous materials
